Abstract
In despite of that there have been much trials to combat with cancer applying newly discovered anti-cancer drug and radie-isotope recently, the choice of radical treatment of carncinoma of the stomach still relies upon early surgical resection.
Present study attempts to clarify the relationship between the five-year survival rate and the type of operation, duration of symptoms, metastasis to regional lymphnodes, histological malignancy and infiltration to the serosa.
Among 221 cases excluding 7 cases of operative death out of 228 operated cases during 8 year period from January 1955 to December 1962, 124 cases (56.1%) were able to follow up for postoperative survival rate. In calculating five-year survival rate, only 62 cases (64.6%) out of 96 cases those who were operated on before September 1958, were selected and they were all able to do follow-up study completely.
There were 8 five-year suuival cases, all of them belonged to the subtotal gastrectomized group.
The percentage ratio of five-year survivors to the total operated cases was 12.9% .
Five-year survival rate among all gastrectomized cases was 22.2%, and it was 12.0% in the positive regional lymphnode metastasis group, while 45.5% in the negative cases.
In the all subtotal gastrectomized group, five-year survival rate was 24.2% and in the case of positive regional lymphnode metastasis 13.6% and in negative 45.5%.
There was no five-year survivor in the total gastrectomized group.
In the unresected group 40% of the cases died within three months and over two third within six months, and only 4 cases survived more than a year but not longer than one and a half year.
There was no noticeable difference in the average survival perid between the group who had by-pass procedure and exploratory laparatomy or biopsy group, former being 4.8 months and later 4.5 months.
As the influencing factors in the end-results of surgical resection of carcinoma of the atomach, better end-results were proved to to obtained in the group of shorter duration of the symptoms, negative regional lymphnode metastasis, less malignancy by Broder¢¥s grading and intact serosa.
